Medical Reports and Requests

Any requests for medical reports/letters could take a minimum of 3 weeks to complete as this is classed as private work.

All medical reports/letters must be put in writing and the Secretaries will inform the patient of the fee to be paid before completion of the report

With certain exceptions written within their contract, doctors do not have to carry out non-NHS work.

If we are inundated with NHS work and unwell patients, we may not be able to carry out your requested work.

GPs can only write letters based on fact, e.g., seen when and where and with what symptoms or diagnosis.  We are unable to give opinions on whether a particular condition might affect your ability to carry out tasks or may require changes in your social situation.  This is because we do not have training in that area of expertise, and you would need to see a specialist for that type of report.  Letters requesting that type of information will be rejected.

  • Insurance Claim Forms/Cancellation Holiday: £50.00
  • Ofsted Forms: £50.00
  • University Forms: £50.00
  • GMP Firearms/Shotgun Certificate: £50.00
  • Full Medical Examination: £113 (HGV, Taxi (not Salford), Pilot)
  • Full Medical – Salford Taxi: £90.00
  • Full Medical – Fostering/Adoption: No fee to be taken

Medical Reports and Private Letters

All medical reports and request for letters must be put in writing and passed directly to Secretaries, no fee is to be taken by reception at the time of request.

Secretaries will contact the patient and inform the patient of the fee, once the fee is received the report/letter request will be passed to the GP to be completed.

Secretaries will document on the patient records the fee that has been requested, when the patient brings the fee to reception please give the patient a receipt, log on patient records fee has been paid and task Secretaries to inform the fee has been paid.

Secretaries will then contact the patient to inform the report/letter is ready to collect.

Medicals – (HGV, Taxi etc)

Fees for Medicals e.g HGV, Taxi’s etc.. should be paid on arrival for medical appointment.  Please give patient a receipt and document on patient records fee has been taken for the medical.
